How to administer a DFCS Survey

Getting to the Survey

In order to administer a DFCS survey, we'll first need to navigate to the Survey Form.
To do this, start my opening the navigation menu, in the upper left-hand corner of any page.

Then select survey from the menu options.

This will bring you to the Case Search page. (Note that the icon in the upper right hand should be for Surveys)

From this page, enter the search criteria for the case you're wishing to perform a survey for and select the correct result from the list.

This will bring up the case info screen, so that you can confirm the correct case. To continue, select "Survey" from the top ribbon of icons.

This will bring up the list of survey questions.
-At this point, please pass the device to the client to complete the form.-

Taking the Survey

Please note, the appearance of the form can be different from device to device, but the overall functionality will remain the same.

Currently, questions on the form come in two varieties: Multiple Choice or Text.

  • Multiple Choice
    • To complete a Multiple Choice question, press the blue text "Please Select" directly below the question.
    • This will bring up your device's "drop down menu" to choose your answer.
    • Note: Some devices may require you to select a "Done" option on this menu.
  • Text Entry
    • To enter text fields, touch inside the text box below each question.
    • This will bring up your device keyboard for typing.
    • Note: Special characters, such as 'Emoji' may not function correctly in these fields, and should be avoided.

Completing the Form

After all of the questions are completed, you'll need to capture the Client's Signature, the Signature and name of any third party who filled out the survey on behalf of the client, and the names of any witnesses present when the survey is completed.

In order to capture the client's signature, press the "Get Signature" button. This will open a special dialog that allows a finger or stylus to trace directly on the screen, allowing for a true signature capture.

Once inside the dialog, the user can clear the signature by pressing "Clear" in the upper right-hand corner, return to the Survey by pressing "Cancel" in the bottom left-hand, and can save the current signature with the bottom right-hand button.

After saving, the signature will appear on the form.


If the Survey was completed by a third party on behalf of the client, please click the switch to indicate this. Doing so will open a new series of options to capture the name and signature of the third party. This signature is in addition to the client's signature, which is always required.

Enter any witnesses present, separated by  a ','.

When done, press the 'Save' button to save and exit the survey.

Reviewing the Survey

For client confidentiality and survey accuracy, surveys cannot be changed after submission.
